When Sleeplessness Signals Something More Serious

Most people experience occasional sleepless nights, often blaming stress, anxiety, or a busy lifestyle. Muscle twitching and excessive sweating are also commonly overlooked as minor health concerns. But what if these symptoms are signs of a rare neurological disorder affecting the brain, nerves, and autonomic nervous system?

A recent case treated at STAR Hospital, Nanakramguda, highlights the importance of recognizing unusual neurological symptoms early. A 48-year-old man presented with severe insomnia, continuous muscle twitching, excessive sweating, muscle stiffness, and episodes of confusion. What initially appeared to be a combination of unrelated symptoms was ultimately diagnosed as Morvan Syndrome, an exceptionally rare autoimmune neurological disorder.

What Is Morvan Syndrome?

Morvan Syndrome is a rare autoimmune condition in which the body's immune system mistakenly attacks components of the nervous system. The disorder affects multiple systems simultaneously, including the peripheral nerves, brain, and autonomic nervous system, leading to a state of continuous nerve overactivity.

The condition is extraordinarily rare, with fewer than one person per million estimated to be affected worldwide. Since its first description in 1890, fewer than 100 cases have been reported in English medical literature.

Recognizing the Symptoms

Patients with Morvan Syndrome often experience a unique combination of neurological and autonomic symptoms, including:

  • Severe insomnia or near-total loss of sleep
  • Persistent muscle twitching and muscle rippling
  • Painful muscle cramps and stiffness
  • Excessive sweating (hyperhidrosis)
  • Fluctuating heart rate and blood pressure
  • Confusion, hallucinations, or behavioural changes

According to Dr. B. Santhosh Kumar, Senior Consultant Neurology, STAR Hospital, severe insomnia accompanied by muscle overactivity and autonomic disturbances should prompt consideration of rare autoimmune neurological disorders.

The Role of Advanced Diagnostic Testing

To identify the underlying cause, the patient underwent a comprehensive neurological evaluation, including autoimmune antibody testing.

The investigations revealed positivity for CASPR2 antibodies, which are part of the Voltage-Gated Potassium Channel (VGKC) antibody complex. These antibodies interfere with normal nerve signaling, causing excessive electrical activity throughout the nervous system.

Additional investigations included:

  • Whole Body PET-CT Scan
  • Paraneoplastic Antibody Screening
  • Autoimmune Neurological Testing

Importantly, the PET-CT scan and paraneoplastic evaluation were normal, helping rule out associated cancers such as thymoma, which can occasionally occur alongside Morvan Syndrome.

Advanced Treatment Offers Hope

Although historically considered a potentially life-threatening condition, advances in autoimmune neurology have dramatically improved outcomes.

Treatment focuses on suppressing the abnormal immune response and may include:

  • Intravenous Immunoglobulin (IVIG)
  • High-dose corticosteroids
  • Plasmapheresis (plasma exchange)
  • Rituximab and advanced immunotherapy in selected patients
  • Symptomatic management for sleep and autonomic disturbances

Following specialized neuro-immunological treatment, the patient experienced gradual improvement in sleep quality, muscle activity, and autonomic symptoms.

Why Early Detection Matters

Because Morvan Syndrome is so rare, many patients are initially misdiagnosed with psychiatric conditions, anxiety disorders, or primary sleep disorders. Delayed diagnosis can prolong suffering and increase the risk of complications.

Specialists emphasize that persistent insomnia accompanied by unexplained muscle twitching, excessive sweating, behavioural changes, or autonomic symptoms should not be ignored.

Modern antibody-based diagnostics and specialized neuro-immunology services are enabling earlier identification of rare but treatable neurological disorders, leading to significantly better outcomes.

Expert Care for Rare Neurological Conditions

This case highlights how advanced neurological evaluation can uncover uncommon autoimmune disorders that might otherwise go undiagnosed. Early recognition, timely antibody testing, and specialized treatment can be life-changing for patients affected by rare conditions such as Morvan Syndrome.
