Heart Valve Disease: Warning Signs and When Treatment Is Needed

Could breathlessness, unexplained tiredness, or a gradual decline in exercise capacity be more than just signs of aging or the early warning signs of an unhealthy heart

Sometimes, these seemingly ordinary changes can be early signs of heart valve disease. Heart valves help blood flow through the heart in the right direction. When a valve becomes narrowed or starts leaking, the heart may have to work harder to maintain normal blood flow. Because the heart can compensate for these changes for a long time, valve disease may remain unnoticed until symptoms become more obvious.

Why Heart Valve Disease Can Go Unnoticed

The heart can adapt remarkably well when a valve begins to narrow or leak. A person may have few or no symptoms for years even though the heart is working harder. Symptoms may also develop very gradually. A person might start avoiding stairs, walking less, reducing physical activity or feeling more tired than usual without realising that their exercise capacity has changed. As a result, valve disease may sometimes be detected only when symptoms become more noticeable, during a routine medical examination or through a cardiac investigation.

Warning Signs That Should Not Be Ignored

Breathlessness while climbing stairs, fatigue, dizziness and reduced stamina are often attributed to ageing, stress or lack of fitness. However, new or progressively worsening symptoms deserve medical attention.

Significant valve disease may also cause:

  • Chest discomfort
  • Fainting or near-fainting
  • Swelling of the legs
  • Increasing breathlessness
  • Reduced ability to exercise
  • Unexplained tiredness

A heart murmur detected during a physical examination can provide another important clue, although not every heart murmur indicates serious valve disease. Persistent or worsening symptoms should therefore be evaluated rather than automatically considered a normal part of ageing.

How Heart Valve Disease Is Diagnosed

  • Evaluation usually begins with a clinical assessment and an echocardiogram.
  • An echocardiogram can show how well a valve is functioning, identify narrowing or leakage, assess the severity of the problem and determine how the condition is affecting the heart chambers.
  • If an intervention is being considered, additional tests may be required. Transesophageal echocardiography and cardiac CT can provide detailed information about the valve and surrounding structures, which can be important for treatment planning.
  • In selected patients, cardiac MRI, stress testing or cardiac catheterisation may also be recommended.
  • Modern valve care therefore involves not only identifying the valve problem but also understanding its effect on the heart and assessing the patient's individual anatomy.

When Is Treatment Needed?

A diagnosis of valve disease does not automatically mean that a procedure is required. Some patients can be monitored safely through regular clinical assessments and echocardiograms. Others may need treatment when the disease becomes severe, symptoms develop, or there is evidence that the heart is being affected.

Doctors consider several factors before recommending intervention, including:

  • Type and severity of valve disease
  • Presence and severity of symptoms
  • Heart function
  • Valve anatomy
  • Age and overall health
  • Other medical conditions
  • Procedural risk
  • Expected long-term benefit

The important question is not simply whether a valve is abnormal. It is whether the disease has reached a stage where treatment is likely to provide greater benefit than continued observation.

How Treatment Options Have Expanded

  • Structural heart treatment has changed significantly over the past decade. Treatment is no longer limited to conventional open-heart surgery for every patient.
  • For appropriately selected patients with severe aortic stenosis, Transcatheter Aortic Valve Replacement, or TAVR/TAVI, can replace the diseased aortic valve through a catheter-based procedure.
  • Treatment options have also expanded for other valves. Transcatheter Mitral Valve Replacement, or TMVR, may be considered for selected patients with significant mitral valve disease. Transcatheter Edge-to-Edge Repair, or TEER, can provide a catheter-based treatment option for selected patients with significant mitral or tricuspid regurgitation.
  • For severe tricuspid valve disease, Transcatheter Tricuspid Valve Replacement, or TTVR, represents an important emerging area of structural heart treatment.
  • These advances have expanded the range of options available, particularly for patients who may be unsuitable for conventional surgery or who have a higher surgical risk.

Is Minimally Invasive Treatment Always Better?

Not necessarily. A transcatheter procedure is not simply a less invasive alternative to surgery that is suitable for everyone. The choice of treatment depends on the valve involved, severity of disease, anatomy, heart function, age, associated health conditions, procedural risk, and expected long-term benefit.

For some patients, conventional surgery may remain the most appropriate option. For others, a transcatheter procedure may offer a safer or more suitable approach. The goal is not to choose the least invasive treatment. The goal is to choose the most appropriate treatment for the individual patient.

Why the Heart Team Matters

  • Structural heart disease is not a one-size-fits-all condition. Two patients with apparently similar valve problems may have very different anatomy, health conditions and procedural risks.
  • A multidisciplinary heart team can bring together clinical assessment, advanced imaging, interventional cardiology and cardiac surgical expertise before recommending a treatment strategy.
  • This is particularly important in complex cases where doctors need to determine whether intervention is required, which valve should be treated, which technique is most suitable and when treatment should be performed.

Can Valve Disease Be Present Without Symptoms?

Yes. Some people gradually adapt to a reduction in their exercise capacity and may not recognise that their physical abilities have changed. Others may have significant valve abnormalities discovered incidentally during a medical examination or cardiac investigation.

However, having no symptoms does not automatically mean that treatment is required. Doctors assess the severity of the valve disease, its effect on the heart and whether it is progressing before deciding between continued monitoring and intervention.

Why Timing Matters

Timing is one of the most important aspects of modern heart valve care. If severe valve disease remains untreated for too long, continued pressure or volume overload can place increasing stress on the heart. Over time, this may contribute to heart failure and other complications. On the other hand, treating too early can expose a patient to procedural risks when the expected benefit may be limited. The aim is therefore to identify the appropriate window for treatment, before significant and potentially irreversible damage develops, while ensuring that the expected benefits of intervention outweigh the risks.

The Right Treatment at the Right Time

  • Heart valve disease can remain relatively silent while gradually affecting the heart. Breathlessness, unexplained fatigue, dizziness, chest discomfort or a decline in exercise capacity should not automatically be considered a normal part of ageing.
  • Early evaluation does not mean that every patient will need a procedure. It means understanding the condition, monitoring its progression and identifying when treatment is most likely to provide meaningful benefit.
  • With advances in imaging, heart-team decision-making and transcatheter treatments such as TAVR, TMVR, TEER and TTVR, patients now have a wider range of treatment options.
